Getting Sick: Disease and Treatment – In medieval fantasy, making health a concern is a way to make a game grittier and more grounded. This is a procedure which details how player characters can become infected with a disease, how diseases can develop, and how non-magical treatment can be provided.

Lycanthropy in Player Characters – There’s a trickiness when it comes to lycanthropy (or other monstrous manifestations like vampirism) and your player characters: make the disease too awful (as above) and it becomes a “gotcha” annoyance, or make it too powerful and with too few drawbacks, and you’ve suddenly got a whole posse of adventurers beating every bush in the neighborhood hoping to scare out a lycan so they can be bit, and thereby become super powered.

Magical Substance Addiction  – In a world of might and magic, problems of the real world seem mundane. But in a world of might and magic, why not make mundane problems more fantastic? Enter the Dust Cabal.

Potent Potions Pontifications – There should be more to quaffing a potion than the threat of running out of the precious liquid. It’s high time to get the routine out of the potions. There should be consequences for drinking potions. Logically, the more exposure one has, the more extreme side effects.

Sinister Swag: Unsettling Items – This is a collection of six magic items that give loot an unexpected sting, and a d100 (50 entry) table of creepy clutter to give environments an unsettling atmosphere. Use them as they are, chop them up, or use them as templates to create your own material. There’s some guidance at the end for creating these kinds of risk vs. reward magic items.

Twisted Changes: Corruption and Mutation – A cornerstone of dark fantasy is supernatural influences infecting and changing people. From the gothic tropes of lycanthropy and vampirism to the trappings of Lovecraftian horror, the common idea is that people can be changed in unexpected ways (often against their wishes). This article is twenty ideas for how corruption can manifest and mutate player characters or NPCs.
